2023 DKMB Coaching Staff Evaluation Form

All families involved in DKMB Minor Baseball are encouraged to provide feedback on the coaches and assistants for their teams.  This feedback will help the association work at ensuring that all players experience rewarding seasons in continuing years.

Please submit one evaluation per coach or assistant that you wish to provide feedback for.

Please note that this is a confidential survey and only the coaching committee will have access to this information.  These coaches/assistants may be provided with aggregate results from all scale (1 to 5) evaluations provided.  Specific comments will not be shared with coaches/assistants. The information provided will help DKMB determine future coaching positions.
